We are not in network with Aetna from the Marketplace.  

 

Avmed:

State employee or any employer plan

We no longer take Avmed from the healthcare.gov website

 

Florida Blue:

Any Blue Card from any state.  We file claims with Florida Blue

Any Anthem from any state

We do accept Blue Select from healthcare.gov

We do not accept My Blue HMO

 

Florida Health Care Plan is not Florida Blue, and we do not accept Florida Health Care Plan.  The logo looks like Blue Cross but is not accepted in

 

Florida Hospital / Advent Health

We are in network with Advent health insurance plans including

HFHP comm, FHHS, AH Employee.

We are not in network with Cigna Sure Fit, UHC Nexus, Oscar Health.

 

Cigna:

Any Cigna employer plan

We do not accept Cigna EPO or Sure Fit

We do not accept Cigna from healthcare.gov- will not allow us in network

 

Humana:

We are in network with Humana

We do not accept Humana Gold Plus

 

United Healthcare:

Any United plan including Neighborhood, Student Resources, UMR

We do not accept United Healthcare from healthcare.gov

We do not accept Community plan

We do accept Surest Plan, United Healthcare Choice Plus. 

 

Medicare:

We accept Medicare part B and will file secondary insurance

We are out of network with all Medicare Advantage plans but coverage for in network and out of network often means only slightly higher copay will still pay claims.  Look at Out of Network benefit to determine coverage.

 

We do not file any work-related claims or auto accident claims.

We are not in network with Oscar, Ambetter, Bright Health, Medicaid plans
